Population Health Collaborative Quality Initiatives (PO Participating CQI's)

    Focus on improving outcomes in areas such as chronic disease management, prevention, population-level health improvements, and health equity—rather than on acute surgical or procedural care. These CQIs are claims-based, clinical data initiatives available to primary care providers and select specialty practices. Practitioner participation is coordinated and managed through their Physician Organization (PO)
